Relationship between donor-selection criteria and the pathological examination

Abstract
The aim of the study is to present the relationship between the initial pathological evaluation of a donor's liver and the evaluation based upon clinical and biochemical criteria. Five liver extractions were performed at the Warsaw University School of Medicine Department of General Surgery and Liver Diseases, with a view to the organs' transplantations. The pathological evaluation was based on biopsies conducted right after the harvesting and the graft's prefusion in the University of Wisconsin (UW) solution, and - in cases accepted for transplantation - on the "zero biopsy" conducted after the reperfusion of the grafts. Extemporaneous as well as quick hematoxylin and eosin (H&E) stainings were used. Two cases were accepted for transplantations. In one of the cases a discrepancy between the clinical and the pathological evaluation has appeared. The pathological examination influenced the decision to proceed with the transplantation. One liver was rejected due to negative results of both biochemical and pathological tests, and the clinical criteria. Two livers were rejected based only on biochemical results and the macroscopic picture of the liver following the perfusion in the UW solution.
