** Multivariate analysis odds ratio: adjusted for small for gestational age (SGA – defined as neonatal weight 37 years, maternal body-mass-index (BMI) >30 kg/m, in vitro fertilization (IVF) and gestation (22 0/7 to 27 6/7 vs 28 0/7 to 31 6/7 weeks).
