Transesophageal echocardiography is of great value in the assessment of proximal coronary anatomy. This study presents a case of non-invasive diagnosis of an anomalous right coronary artery prior to coronary catheterization. During transesophageal echocardiography, the right coronary artery was found to originate from the left Valsalva sinus. The artery passed between the aorta and pulmonary trunk, but no flow disturbances had been observed in the proximal segment of the vessel. This was accompanied by significant proximal left anterior descending artery stenosis. The above-mentioned findings were subsequently confirmed by coronary arteriography.
