The authors describe case report of symptomatic cerebral ischaemia treated by surgical non-anatomical vascular grafting. In patient, after a stroke, arteriography revealed left common carotid artery occlusion, right internal carotid artery occlusion and back-flow ('steal phenomenon') from left internal carotid artery to left external carotid artery. Due to numerous additional conditions as: coronary artery disease, respiratory insufficiency, frequent asthmatic attacks, nodular goitre, cataract, essential hypertension, coagulation disturbances, the simplest operation was performed. PTFE vascular bypass between patent right common carotid artery and the point of left common carotid artery bifurcation (also patent) was performed.
