Acute necrotizing pancreatitis (ANP) bears the potential risk of subsequent bile duct damage and stricture which require surgical reconstruction. The authors present the associated difficulties and their management of this complication in two patients. Hepatic duct reconstruction with Roux-en-Y loop was attempted 9 and 24 months following the onset of acute pancreatitis. The good functional outcome of the anastomoses 3.5 and 4.5 years after surgery suggests a prognosis for permanent recovery.
