Presented here is a case of a 19-year-old student with a form of cavitating myelinoclastic transitional sclerosis that had been preceded by an influenza-like infection. The onset of the disease was acute, ran a severe course and at first presented great diagnostic difficulties. The administration of tetracosactide resulted in a dramatic improvement. In addition to typical demyelinization foci, the MRI demonstrated disintegration of the white matter into small and larger cavities. Follow-up MRI examinations suggested that an active demyelinization process was still in course.
